HOW TO RECEIVE YOUR $50 MAIL-IN REBATE
  1. Purchase any qualifying Cuisinart 5.5Qt Stand Mixer (Qualifying Models: SM-55BCC, SM-55C and SM-55BKC) between November 1 – December 31, 2011.
  2. Complete this official Rebate Form with your name & address. Attach the original UPC symbol from your Cuisinart 5.5 Quart Stand Mixer package and a copy of / or your original dated cash register receipt, circle your purchase and mail, with sufficient postage to: Cuisinart Stand Mixer $50.00 Rebate Offer, P.O. THIS IS NOT A STORE COUPON: Submit this rebate form completely filled out with a copy of your sales receipt or invoice and original UPC label from the product package.
